I normally use Owely - it's cross-platform (Windows, OS X, Linux) and allows for annotations (drawing + text). 50MB free (which is plenty unless you're a heavy user), and 1GB extra for $12/year. It's purely a hosted service, though, so won't be suitable if you want to host your images yourself...

I suspect the "blatant advertising" remark was in reference to the vpsBoard links, rather than BuyVM (seeing as it seems another vpsBoard-related post was removed). Not sure though.

Perhaps a better option is to use Gravatar, but turn off the generated defaults. It should be possible to set a single fixed "default" image for people without an avatar. I should also point out that if you're "privacy-minded", you should probably not be registering here with your normal e-mail...
