Here are my requirements:

Any virtualization where I can install OpenVZ on it (i.e. KVM, Xen, VMware, etc...)

1GB of RAM

15GB of Disk Space

At least 2 CPU Cores (the more the better)

100GB Bandwidth on 100Mbps port of faster

At least 2 IPv6 Addresses (native, not tunneled)

Located in Missouri or Illinois

Network uptime as close to 100% as possible

My budget is $10-15 per month.

Also, please don't recommend Vultr.

The affordable Xen/KVM choices in IL/MO aren't that great: Ubiquity's cloud, BudgetVM Xen, Dewlance

edit: take that back, there is one good one in Chicago: CorgiTech (VMWare)

Virpus was in KC and offer Xen, but not sure if they are still offering new services there (looking at their site it looks like the offers are for Seattle).  Although, they are single-homed to Cogent in KC, so that may be a consideration.

Not much in Kansas City, oddly.  Even OpenVZ is barely there... Way less providers than should be.

Chicago is the market likely.

joesdatacenter.com has some dedis starting at $16/month, almost kimsufi-like pricing.  Might not have enough oomph for you though.  The more expensive ones would.
