If you are interested in 2 factor authentication take a look at phonefactor. Usable with sshd and websites, even rdp and the phonecall is free in many countries.

You should have those values from your host/provider of the dedicated server.

Have you checked if it's logging visitors at all or if the problem is when awstats reads the logs? I suspect that it's not even logging at all.
